Service Description

This is an all encompassing package option that has three additional options built into it. These options apply team sports only. For Sports Agency needs, see separate "Sports Agency Plan". 3 month 6 month 12 month Options are available depending on need. Within these options, I will provide injury management consulting for your organization starting from initial injury, diagnosis, treatments, surgery (if needed) and rehabilitation all the way to return to play for each individual athlete that may require such services. These options are based on monthly retainer fees, which are determined based on several factors (including but not limited to the particular sport, # of athletes, length of season, contact vs non-contact sport, and athlete time loss to injury, etc.). Researched backed data can be provided at client's request. *** This plan includes any educational information and research material (peer reviewed literature) as requested. ***This Plan DOES NOT include any travel or lodging expenses which will be invoiced separately if services are required ***
